Visiting Nurse Health System offers Palliative Care through the Advanced Illness Management (AIM) program. It is designed to bring the best quality of life to patients with serious, chronic or life-limiting illness. Yet, this individualized form of care may or may not apply to those who are seeking additional curative treatment. That means hospice can be part of our AIM care, but not all AIM participants are terminally ill.

The AIM program partners with the patient’s physician through every stage of the disease progression. The goal is to prevent pain and suffering as much as possible. It’s also a holistic approach: not only treating the physical symptoms, but the emotional and spiritual challenges, as well. That’s why we combine active and compassionate therapies administered by an entire team of experienced professionals and volunteers who support both the patients and their families. Our interdisciplinary approach includes:

  • RN Care Specialists
  • Rehabilitative Services
  • Counseling for access to community resources
  • Home Health Aides
  • 24/7 clinical support
