About Community Care Services Program (CCSP)
Community Care Services Program (CCSP), helps senior adults and people with functional and/or cognitive impairments stay in their homes and communities for as long as possible. CCSP is a program of the Georgia Department of Community Health. Visiting Nurse Health System provides case management oversight for over 2,000 CCSP clients in the Greater Atlanta area at any given time allowing them to access community-based care and retain their independence. The CCSP service options include the following:
Adult Day Program
Available up to five days a week
Daytime care and supervision in an adult daycare center
Therapeutic activities and outings
Meals adapted to prescribed diets
Dementia-specific care
Alternative Living Services
Alternative residence for persons unable to remain independent in their homes
Meals, medically-oriented personal care and nursing supervision
Emergency Response Services
24 hour, 7 day in-home electronic support between the client and a medical control center
Personal Emergency Response System
Home Healthcare Services
Skilled nursing services—physical, speech & occupational therapy, home health aide, medical social services and medication management
Medical case management
Respite Care
In-home & out of home (where available)
